REQUIREMENTS:
LICENSES & APPROVALS:
_ Copy of ALL Contractor(s) / Qualified Business License
_ Copy of all contractors licenses (Must be active & in good standing)
_ Business Tax Receipt (Must be active & in good standing)
_ Company name (Must be current & active with the Florida Department of State. NOT fictitious name)
_ Condominium Association Certificate of Approval before commencement
INSURANCE:
The insurance certificate MUST list the Association and management as "Additionally Insured" (AI) on ALL certificate(s). The insurance certificate MUST list the Association and “Kinetic Property Management, LLC” as Additionally Insured in the description section.
The Additionally Insured description section MUST say “Certificate Holder is included as Additional Insured in accordance with the policy provisions of the General Liability policy. A Waiver of Subrogation is granted in favor of Certificate Holder in accordance with the policy provisions of the General Liability and Workers’ Compensation policies.” This also applies to subcontractors.
COVERAGE REQUIRED:
General Liability insurance (minimum $1,000,000).
Workers Compensation insurance (minimum $1,000,000).
OTHER REQUIREMENTS:
Complete the Contractor, Subcontractor Registration Form (see below). The name and contact information for the General Contractor, Subcontractor(s). Once approved, the Association will issue a Certificate of Approval with the Association’s seal & notarized.
_ Acknowledgment, Release, and Waiver of Liability, Contractor rules must be signed by all
employee(s), contractor(s), company owner(s) & unit owner(s).
_ Copy of ALL City/County permit(s) must be submitted
_ Provide drawings of proposed remodeling work to be completed
_ Provide before, during, and after photos of all work
_ Provide a copy of ALL closed permits upon full completion of work
_ Must post ALL permits on the unit door & have filed a notice of commencement with the County
before any commencement of work (as required by law).
_ Provide the Association with a copy of all documents & drawings submitted to the city and
any updated(s) or changes that may occur throughout the project
_ Any changes may need to be approved by the association first
_ All contractors MUST have general liability & workers compensation insurance (other insurance may be required)
ALL INFORMATION MUST BE SUBMITTED TO THE ASSOCIATION’S MANAGEMENT FOR REVIEW PRIOR TO COMMENCEMENT. FAILURE TO COMPLY WILL RESULT IN $100 PER DAY FINES PLUS LEGAL FEES TO UNIT OWNER.
The below list is the minimum requirements and more information, documents, etc may be required. After receiving your request, we will notify you if there are more requirements for the property you are requesting to perform work at. Each contractor or subcontractor needs to complete and submit the registration form, insurance certificate, and all other required documents.
Processing time: It will take at least 1-2 business days to review the request and in some instances, it may take longer. We will issue a response as soon as possible.
